Failure Mode Analysis

Definition

Failure Mode Analysis (FMA) is the act of determining the physical failure mechanism that results in the functional failure of a component or equipment. This includes identifying how equipment can fail, why it might fail, and how best to address potential failure, and it may or may not include criticality.

How does iReliability™ enable this?

There are a number of FMA methodologies that can be used to create maintenance strategies capable of detecting and dealing with defects. iReliability™ utilizes a proven methodology that incorporates the Asset Health Matrix and RCM Blitz®.

Asset Health Matrix (AHM) / Equipment Maintenance Plan Tool

Streamlined Failure Mode Analysis with appropriate tasks mapped to failure modes
The iReliability™ Asset Health Matrix/Equipment Maintenance Plan tool provides a starting point for FMA by utilizing the most common failure modes in asset components and parts to identify which mitigating actions or defect identification strategies could apply to the asset and establish a baseline. It further assists in the ongoing implementation of a failure mode-driven maintenance strategy by validating the current program for the balance of the plant – those assets not included in RCM analysis based on criticality.

With an extensive failure mode library assigned at the part level, all failure modes have assigned PdM, PM, and lubrication activities already mapped. Users can apply inclusion/exclusion rules to develop a highly customized equipment maintenance plan that is based on the engineered failure modes in your plant, enabling the quick design of equipment maintenance plans. This tool also identifies tasks that could be performed by task-qualified operators, sometimes referred to as “Basic Care”, and utilizes asset criticality to determine maintenance strategy.

RCM Blitz®

Perform RCM analyses faster while simultaneously generating, communicating, and tracking RCM work requests
Design and implement a failure mode driven reliability strategy and return results in a fraction of the time of traditional RCM methods via the convenient and easy-to-use RCM Blitz® wizard. Using the asset criticality ranking index , the top 5-20% of the equipment will receive a complete RCM analysis. Following analysis, work requests are generated to ensure effective communication and tracking of RCM results and individuals are assigned specific tasks and durations for participation and implementation. This ensures that RCM is not only performed, but implemented. Viewed as traditional RCM methodology, RCM Blitz® is fully compliant with the SAE RCM Standard JA1011 while engineered to be quick and extremely effective.
